Difficulty breathing, chest pain, and cough key symptoms for embolism

ScienceDaily (Oct. 22, 2012) ? Difficulty breathing, chest pain, and cough may sound like symptoms of a heart attack; however, new research shows that these are the key symptoms for pulmonary embolism (PE), a potentially fatal blockage in the lung.

A research team from Saint Vincent's Medical Center in Bridgeport, Connecticut, retrospectively reviewed the clinical features, risk factors, and ECG findings in 334 patients with confirmed PE. Results showed that dyspnea or difficulty breathing, chest pain, and cough were present in 72%, 38%, and 19% of the patients, respectively, and dyspnea was the only presenting symptom in 29%.

Cancer was the most common risk factor present in 27%, followed by prior history of deep vein thrombosis or PE, immobilization, and surgery in 19%, 15%, and 15%, respectively.

This study was presented during CHEST 2012, the annual meeting of the American College of Chest Physicians, held October 20 -- 25, in Atlanta, Georgia.

(AP) ? U.S. Rep. Jesse Jackson Jr., who covertly went on medical leave months ago and has given no sign of when he'll return to work, seems to be in a "fragile state" and is heading back to the hospital where he was treated for bipolar disorder, a fellow Democratic Illinois congressmen said Monday.

U.S. Rep. Bobby Rush said Jackson looked well when he visited him at his home, but that he was going back to the Mayo Clinic in Rochester, Minn., on Monday for what could be another extended stay. Jackson's father, the Rev. Jesse Jackson, has characterized the Mayo visit as a checkup that could lead to further inpatient treatment.

Jackson quietly went on medical leave in June, and his office released little information until confirming he was being treated at Mayo for bipolar disorder and gastrointestinal issues. He was released in September and returned to his Washington home.

But he has neither campaigned nor appeared publicly for his Nov. 6 re-election aside from a robocall released to voters over the weekend. In it, he said he was "anxious to return to work" but that it was against his doctors' advice and asked for patience. His spokesmen didn't return requests for comment Monday.

Rush told The Associated Press that he and fellow Illinois Democrat U.S. Rep. Danny Davis talked with Jackson at his Washington home for roughly an hour on topics ranging from his mental health to the Chicago Bears. Both he and Davis urged the public for patience and understanding, and they planned an evening news conference.

"He looks well, but he doesn't feel as well as he looks," Rush said. "We have to give him some time and space to heal. He will then again rise up to the level of what he's capable of. Right now he's in a fragile state."

The congressmen decided to visit Jackson, a fellow member of the Congressional Black Caucus, after talking with his family. All three congressmen have similar constituents: Their Chicago area districts are mostly Democratic with many black voters, and Rush took several months of medical leave in 2008 to undergo treatment for a rare form of cancer.

Rush said part of Jackson's healing has been to spend time with his two children, who go to school in Washington. The family also has a home in Chicago, where Jackson's wife, Sandi, is a city council member.

Rush added that Jackson was preparing for an extended stay away from his children at Mayo again, but he did not know further details.

Mayo spokesman Nick Hanson said Monday that Jackson is not a current Mayo patient and declined to give further information.

Jackson's family has said the congressmen remains under doctors' care and will not return to work until he gets clearance from them to do so. But he remains on the Nov. 6 ballot.

Rush, who is also a minister, said he and Davis encouraged Jackson to take his time in getting better instead of focusing on campaigning.

"He expressed a great deal of concern for his family, a great deal of concern for his constituents," said Davis, adding that Jackson must have time to heal so that his future decisions are made after "good solid mental and emotional healing."

Jackson, first elected to Congress in 1995, is largely expected to win re-election next month, but the handling and timing of his medical leave has invited criticism. Some voters and political opponents have called on him to answer more questions and give the public regular updates.

Jackson also remains under a House Ethics Committee investigation for ties to imprisoned former Illinois Gov. Rod Blagojevich. The committee is looking into allegations that Jackson was involved in discussions about raising money for Blagojevich's campaign in exchange for an appointment to President Barack Obama's vacated U.S. Senate seat. And the announcement of the leave came just days after a former fundraiser connected to those allegations was arrested on unrelated federal medical fraud charges.

Jackson has denied wrongdoing.

More recently, the Chicago Sun-Times reported that there is a federal investigation related to his campaign finances.

Jackson vigorously campaigned earlier this year when he faced his most credible Democratic challenger of his career, former U.S. Rep. Debbie Halvorson, but still captured a strong majority.

Rush said Jackson needs time to heal.

"It's almost like Jesse Jackson Jr. is the Derrick Rose of the Illinois delegation," the congressman said, referring to the Chicago Bulls star who is recovering from surgery on a torn ACL.

"The Bulls, the coaches, the owners, they're not trying to push Derrick Rose to come back before he's ready, to come back before he's fully healed."

Thanks to an anonymous tipster, Autoblog has learned that the so-called "emergency refresh" of the Honda Civic is just around the corner. In fact, the automaker is already urging its dealers to clear out remaining 2012 model year stock because "The changes made to the 2013 model will make the outgoing 2012 Civic a difficult model [to] sell when they are side to side." Those are the words of David Hendley, Assistant Vice President of Honda National Sales excerpted from a recent letter to dealers. The communiqu? goes on to note that the refreshed Civic will go on sale on November 29 ? a date, we note, that coincides with press days for the Los Angeles Auto Show.

According to Hendley's letter, buyers can look forward to "dramatic improvements to the exterior styling that moves Civic into a more premium sporty direction." The text also promises that the 2013's interior will receive "upgrades and improvements to bring more sophistication and quality to its appearance." For his part, American Honda President Tetsuo Iwamura has previously been quoted as saying that changes will be made to "improve the Civic's drivability," suggesting suspension and powertrain alterations are afoot as well.

The ninth-generation Civic has only been on sale for a year, and such a substantial refresh this early in the generation's lifecycle is unprecedented these days.

The Japanese automaker has been stung by widespread criticism for the 2012 Civic range's lackluster dynamics, lessened interior quality, noise levels, low feature count and uninspiring design ? perceived shortcomings exacerbated by the industry's hyper-competitive compact car segment. Among the critics, the Civic was given poor marks in a review by this site as well as influential institute Consumer Reports, which went so far as to remove the Civic from its coveted Recommended list. In fact, CR editors labeled the 2012 Civic a "car to avoid" in August.

A lack of critical praise doesn't seem to be harming Civic sales, however. According to the automaker's own sales data, the 2012 model is performing quite handsomely, having sold 234,029 units through September of this year, with 21,546 of those units coming last month alone. That means Honda is shifting almost 40-percent more units this year than the (admittedly aging) eighth-generation model did in 2011. The Civic is now selling near the top of its class, and Hendley notes that the company's Indiana plant will be running at full-tilt to build the updated 2013 model ? in fact, he indicates that the US will even draw units from Canadian production.

When contacted for comment about the Hendley dealer letter, Sage Marie, Honda's Senior Manager of Public Relations, confirmed to Autoblog that the automaker will have Civic news to reveal next month, noting that Honda is keen "to ensure Civic remains the benchmark in the segment and remains as competitive as it can be."

For your dealership to take full advantage of the refreshed Civic and the marketing we will have supporting it you must greatly increase the turn rate on our 2012 Civics. With the unprecedented enhancements we have made to the current Civic after its first year in its cycle you will want to have little to no Civics on hand at arrival time of this refreshed 2013 model. Deal to push Russian oil giant's output past Exxon's

MOSCOW (AP) ? Russian state-owned oil giant Rosneft strengthened its hold on the country's lucrative oil industry when it sealed a deal Monday to buy TNK-BP, the 50-50 joint venture between BP, the British energy country, and a group of Russian oil oligarchs.

The deal will allow Rosneft, already the country's top oil producer, to increase its global profile. The new combined company will leapfrog ExxonMobil Corp. to become the world's largest publicly traded producer of oil and gas, in terms of output ? ExxonMobil's latest earnings show its daily output at 4.2 million barrels of oil, below the expanded Rosneft's projected 4.6 million.

As part of the deal, BP succeeds in unloading its 50 percent stake in its troublesome joint venture and in return gets $17.1 billion in cash from Rosneft and a 12.84 percent stake in Russia's biggest oil company. The British company is also planning to use some of the money it's reaped from the sale to raise its stake in Rosneft to 19.75 percent. BP will also get two seats on the Russian company's nine-member board.

In a parallel development, Rosneft said it had agreed to buy the other 50 percent in the joint venture from AAR, for $28 billion, and that the deal was ?entirely independent of the transaction with BP.? AAR confirmed that a memorandum of understanding has been signed but added that it is still subject to other conditions.

Rosneft, formed in 1993 out of the former Soviet state oil and gas authority, is currently 75 percent owned by the government. The remaining shares are either traded publicly on the Russian stock market or owned by the company and its managers. It is not yet clear how much of Rosneft's existing shares BP will take or if the Russian company will have to issue new stock to seal the deal.

Rosneft CEO Igor Sechin broke the news about the deal Monday afternoon to President Vladimir Putin, telling him that the deal would be worth about $61 billion.

?This is a good big deal which is important not only for Russia's energy sector but for the entire Russian economy,? Putin said in remarks carried by Russian television.

Russia is an important part of BP's business, accounting for a quarter of its oil production. BP Chief Executive Bob Dudley has looked to the country's vast oil resources as a key ingredient to the company's recovery from the disastrous oil spill in the Gulf of Mexico.

(AP) ? A Wisconsin man who had been accused of domestic violence and slashing his wife's tires took a gun into the spa where she worked Sunday and shot seven women, three fatally, before killing himself, a police chief said.

The shootings set off a confusing, six-hour search for the gunman that locked down a nearby mall, a country club adjacent to the spa and the hospital where the survivors were taken. The search froze activity in a commercial area in Brookfield, a middle-to-upper class community west of Milwaukee, for much of the day. Ultimately, he was found dead in the spa.

Authorities said it would take time to sort out exactly what happened, and emphasized they were still interviewing witnesses and rescuers and did not have a firm timeline of events. At a news conference Sunday night, Mayor Steve Ponto called the shootings "a senseless act on the part of one person."

The chaos started around 11 a.m. at the Azana Day Spa, a two-story, 9,000-square-foot building across from a major shopping mall. The first officers on the scene found the building filled with smoke from a fire authorities believe was set by the suspect, Radcliffe Franklin Haughton, 45, of Brown Deer, Brookfield Police Chief Dan Tushaus said.

They also found a 1-pound propane tank they initially thought might be an improvised explosive device, Tushhaus said. That slowed the search of the building as law enforcement agents waited for a bomb squad to clear the scene.

Tushaus said later that police didn't know whether the gunman brought the propane tank to the spa or it was left by a contractor.

The search also was complicated by the layout of the building, with numerous small treatment rooms and several locked areas, Tushaus said. While officers initially thought the gunman had fled the building, they later found his body in one of the locked areas, he said.

The bodies of the victims were also found in the spa. Tushaus said investigators were still working to identify them. He said the four survivors were between the ages of 22 and 40. He didn't know if they were employees at the spa or customers, and it wasn't clear if the man's wife was among the victims.

Haughton had recently been arrested after witnesses identified him as the person who slashed his wife's tires, police said.

He appeared in court Thursday. A four-year restraining order was issued, and Haughton was ordered to turn any firearms over to the sheriff's department.

Haughton's father, Radcliffe Haughton, Sr., spoke to a television station and The Associated Press shortly before police announced that they had found his son's body. In telephone interviews from Florida, he said he had last spoken to his son a few days ago, but didn't have any indication anything was wrong. He begged his son to turn himself in.

After learning of his son's death, he said only: "This is very sad."

A sea of ambulances and police vehicles collected at the scene shortly after the shooting. A witness, David Gosh of nearby West Allis, told the Milwaukee Journal Sentinel he was returning from duck hunting with his father and a friend when he saw a woman emerge from the spa, screaming, as she ran into traffic.

"She ran right out into the street was pounding on cars," Gosh told the newspaper. Moments later, a man with a handgun ran out. He appeared to be chasing the woman but then went back inside, Gosh said.

At the hospital where the victims were taken, staff members were escorted inside during the temporary lockdown and critically injured patients were accepted with a police escort. Officers were stationed at entrances.

The hospital released a statement saying two women had undergone surgery, and one was in critical condition. Another was expected to have surgery Sunday night.

The shooting investigation and manhunt paralyzed a normally bustling shopping district. Inside the mall across the street from the spa, people waited patiently for updates. Gina Kralik, a bartender at Red Robin Gourmet Burgers, said people had been allowed to leave at one point but then police had decided not to let anyone come or go from the mall.

"We're just sitting watching the news and also trying to find out what's going on," she said about 3 p.m.

Austin Della, 17, was working at a department store in the mall when he heard announcements over the loudspeaker asking people to move their cars out of one parking lot. The mall was then locked down for almost three hours, he said, and customers joked about the good service they would get as the only clients in the store.

"Everyone was really calm," Della said. "If not for all the announcements, I don't think anyone would have known that anything was happening."

It was the second mass shooting in Wisconsin this year. Wade Michael Page, a 40-year-old Army veteran and white supremacist, killed six people and injured three others before fatally shooting himself Aug. 5 at a Sikh temple south of Milwaukee.

The shooting at the mall took place less than a mile from where seven people were killed and four wounded on March 12, 2005, when a gunman opened fire at a Living Church of God service held at a hotel.

For the abstract, "Lack of Sleep is Associated with Increased Risk of Injury in Adolescent Athletes," researchers asked middle and high school athletes (grades 7 to 12) enrolled at the Harvard-Westlake School in Studio City, Calif., to answer questions about the number of sports they played and the time they committed to athletics (at school and through other programs), whether they used a private coach, whether they participated in strength training, how much sleep they got on average each night, and how much they subjectively enjoyed their athletic participation. Seventy percent of the student athletes (112 out of 160 students; 54 males and 58 females; mean age 15) completed the survey, conducted in conjunction with Children's Hospital Los Angeles. Researchers then reviewed those students' school records pertaining to reported athletic injuries.

Hours of sleep per night was significantly associated with a decreased likelihood of injury, according to the study results. In addition, the higher the grade level of the athlete, the greater the likelihood of injury -- 2.3 times greater for each additional grade in school. Gender, weeks of participating in sports per year, hours of participation per week, number of sports, strength training, private coaching and subjective assessments of "having fun in sports" were not significantly associated with injury.

"While other studies have shown that lack of sleep can affect cognitive skills and fine motor skills, nobody has really looked at this subject in terms of the adolescent athletic population," said study author Matthew Milewski, MD.

"When we started this study, we thought the amount of sports played, year-round play, and increased specialization in sports would be much more important for injury risk," said Dr. Milewski. Instead, "what we found is that the two most important facts were hours of sleep and grade in school."

The advanced age risk may reflect a cumulative risk for injury after playing three or four years at the high school level, Milewski said, and older athletes are bigger, faster and stronger.
